This webinar will showcase a pioneering project in Thurrock, where North East London NHS Foundation Trust integrated a lead pharmacist into a nurse-led community frailty service. Over 12 weeks, the pharmacist improved prescribing practices, introduced ambulatory blood pressure monitoring, and aligned treatment goals with frailty status—reducing harmful polypharmacy and enhancing care for older adults.
The project led to a 31% reduction in anticholinergic burden, £6,000+ in drug cost savings, and safe deprescribing of 88 medications. Over 90% of patients were transitioned back to GP care, easing nursing workloads and potentially reducing hospital admissions.
