Insights from a pharmacist-led intervention to reduce long-term opioid use

Workshop

This workshop will present important new research to develop and test a practice pharmacist-led intervention (PROMPPT) for people prescribed long-term opioids for persistent pain. The PROMPPT intervention, comprising a personalised review and an associated pharmacist training package, aims to reduce opioid use without making pain worse.

The workshop will highlight how PROMPPT was designed, explore patient and pharmacist experiences of the PROMPPT review and training, and will present the key findings from the just released trial results. Discussion will focus on the implications for clinical practice and next steps.

Practical ways to reduce opioid use without making pain worse

Workshop

Join us for an interactive workshop introducing the PROMPPT intervention – a personalised review and a comprehensive pharmacist training package. Designed to equip pharmacists with skills and knowledge to review patients prescribed opioids for persistent pain, the PROMPPT intervention was developed, in conjunction with patient partners, by experts in pain management, behaviour change, primary care, and medical/pharmacy education.

Grounded in contemporary theory and best practice, PROMPPT takes a person-centred approach based on individual values and preferences in decisions about opioid use. This session will explore how key aspects of the PROMPPT review and training package support pharmacists in guiding individuals through discussions about persistent pain and opioid use, providing insights into the approach and its practical application in clinical practice.

Essential cardiovascular skills: Blood pressure monitoring & on-the-go ECG assessments

Medication safety initiatives to reduce anticholinergic burden

Workshop

This session will detail the successful service development projects that have been undertaken in West Yorkshire to reduce anticholinergic burden. It will illustrate how the work has impacted and led to a decrease in ACB scores across the West Yorkshire Sub-Integrated Care Board.

Managing non-transplant immunosuppressive patients on admission

Workshop

The session will cover what pharmacists need to know when caring for patients admitted on biologics and DMARDs for non-transplant long term immune conditions. Key topics include the importance of a thorough drug history, understanding how these medications are supplied, and recognising and managing important side effects. The workshop will also explore best practices for ensuring continuity of care at discharge, including what information patients need when leaving secondary care. This session is suitable for all secondary care pharmacists working across inpatient wards, rheumatology, gastroenterology, dermatology, and other areas where DMARDs and biologics are routinely used. Collaborative care: Enhancing frailty management through nurse-pharmacist partnerships

Workshop

This workshop presents a pioneering project in Thurrock, where North East London NHS Foundation Trust integrated a lead pharmacist into a nurse-led community frailty service. Over 12 weeks, the pharmacist improved prescribing practices, introduced ambulatory blood pressure monitoring, and aligned treatment goals with frailty status—reducing harmful polypharmacy and enhancing care for older adults.

The project led to a 31% reduction in anticholinergic burden, £6,000+ in drug cost savings, and safe deprescribing of 88 medications. Over 90% of patients were transitioned back to GP care, easing nursing workloads and potentially reducing hospital admissions. Join us to explore how this collaborative, cost-effective model supports sustainable, person-centred frailty care and can be scaled across NHS services.
