About this event

Anticoagulation therapy is recognised as a high-risk area requiring meticulous attention to safety, continuous vigilance, and seamless coordination across all stages of patient care. This webinar is designed to equip pharmacists with practical knowledge and tools to navigate the complexities of anticoagulation management throughout a patient’s journey—from hospital admission to discharge and into the community.

Through a case-based approach, we will highlight key points where anticoagulation safety can be compromised and explore effective strategies to mitigate these risks. The session will also address the unique needs of specialist patient groups, communication challenges during transitions of care, and how to implement robust anticoagulation stewardship practices.

Topics will include:

  • VTE risk assessment and evidence-based management
  • Anticoagulation considerations in maternity and oncology settings
  • Addressing interface challenges and improving care continuity
  • Applying anticoagulation stewardship principles in practice
  • Key learning from HSSIB investigations into anticoagulation-related incidents

Aim

The aim of this webinar is to strengthen pharmacists’ knowledge and confidence in safe anticoagulation management across care settings by using a real-world patient journey to highlight risks, improve transitions of care, and apply anticoagulation stewardship—especially in high-risk groups such as maternity and oncology patients.

Target audience

This webinar is suitable for all levels and sectors of clinical pharmacist who encounter anticoagulants within their practice.

Learning outcomes

After this webinar, attendees should be able to:

  • Identify key risks and safety considerations in anticoagulation therapy across different stages of the patient journey, including interface and high risk groups (maternity/oncology)
  • Apply principles of anticoagulation stewardship and VTE prevention to their care setting
  • Evaluate lessons from HSIB reports and common safety incidents—such as missed doses and high-risk medication errors
