This masterclass will provide updates on national initiatives to improve medication safety and the sharing of local strategies utilised to prevent harm to patients. A focus will be on the Medicines Safety Programme including polypharmacy and transfers of care.
There will also be a focus on strategies to prevent harm in particular the use of metrics – to measure improvements in safety. Attendees will work together in workshops to discuss and formulate action plans to use at their host organisations.
The UKCPA Pain Management Group will deliver a presentation on one of the high-risk medication groups: opioids, and a real case of a Serious Medication Incident review related to opioids. This will provide attendees with an opportunity to reflect on both the practices associated with making opioid use safer and the lessons learned from opioid errors.
This event will be of particular benefit to pharmacists involved in improving medication safety in secondary and primary care.

and improve safe medication practice in their practice.
