NHS England is recommending all Integrated Care Systems set up virtual wards to facilitate patients receiving acute care in their current settings- to avoid hospital admission and to facilitate and expedite discharges from hospital.
This webinar is ideal for pharmacy teams working in all sectors and will look at the role of the pharmacist in supporting Frailty virtual wards and how they play an important role in keeping older patients out of hospital.
The speaker will describe the role of a virtual ward/ hospital@home service in the care of elderly patients and demonstrate how the role of pharmacy is important in the treatment of these patients.
