Care home residents are amongst the most vulnerable older people in our society. A combination of multimorbidity and polypharmacy makes them particularly susceptible to adverse events related to the prescribing, dispensing, administration and monitoring of medicines.
The day will focus on the key role that pharmacy staff in both hospital and primary care settings can play in reducing potential harm to care home residents through ensuring optimal medicines management.
